Terms and Conditions: Membership Validity: ● The membership is valid for one year from the date of purchase. ● Renewal must be done before the expiry date to continue enjoying membership benefits. Doctor Consultations: ● Consultations are subject to the availability of doctors and must be pre-booked. ● Emergency consultations outside of clinic hours may incur additional charges. Diagnostics and Tests: ● Diagnostic tests included in the membership must be conducted at Care All Clinics or designated partner labs. ● Tests must be scheduled in advance and are subject to availability. BP Monitoring and HGT: ● These services are available on-demand during clinic hours and must be recorded in the patient’s file. ● Home BP monitoring devices and glucometers are not included in the plans. Refunds and Cancellations: ● Membership fees are non-refundable. ● Membership cannot be transferred to another individual or family member. Member Responsibilities: ● Members can avail all services at any Care All Clinics location in and around Mumbai. ● Members are required to inform the clinic of any changes in contact details. Termination: ● Care All Clinics reserves the right to terminate membership if the member engages in fraudulent activities or misuses the services. ● In case of termination, no refunds will be provided. Dispute Resolution: ● Any disputes arising from the membership will be resolved amicably through mutual discussion. ● If unresolved, disputes will be subject to the jurisdiction of the courts in Mumbai. By subscribing to our membership plans, members agree to abide by the above terms and conditions, ensuring a smooth and beneficial healthcare experience for everyone involved.
